Step 1: Basic Information

Step 2 — Site Interview (Site Representative)

Site inspector shall interview the property owner, tenant, or a worker associated with the property. If these individuals are not readily available, the site inspector shall, as a last resort, interview agents or representatives associated with the property.

Step 3 — Observations & Potential Concerns

Did you observe any fuel tanks, propane tanks, compressed air and gas tanks/cylinders, or other above-ground storage tanks?
Did you observe any of the following:
Aside from petroleum products that you already identified above, did you observe any other petroleum products in containers?
Aside from substances that you already identified above, did you observe any other hazardous substances?
Did you smell any unusual odors?
Did you observe any pools of liquid?
Did you observe any of the following:
Did you observe any pits, ponds or lagoons?
Did you observe significant stains of soil or pavement?
Did you observe dead or discolored vegetation related to contamination?
Did you observe large open trash piles, mounds or depressions suggesting trash disposal or other solid waste concerns?
Did you observe or were you provided any information from the site contact indicating any of the following:
City Sewer or Septic?
Did you observe any interior heating and cooling systems?
What energy or fuel is used to heat the building?
Did you observe any interior floor drains or sumps?
